We may also store associated information received from other health care professionals as part of your ongoing care.

• To help inform the decisions we need to make about your care

• To ensure that your treatment is safe and effective, including any advice that may be provided as part of your care

HOW THIS INFORMATION IS USED

We may use the above information to produce a report to send to the original referrer or other appropriate health professional and to enable us to order a hearing aid direct from a manufacturer.

In addition to your ongoing hearing/ear care, we may use this information to remind you when appointments are due and suggest relevant products or services that we believe may be of interest. We use your contact information to respond to queries from you. We may occasionally ask you for your feedback on our services and to offer the opportunity to trial new products.

 

PROCESSING, STORAGE AND RETENTION OF YOUR INFORMATION

Your data is stored and processed on password protected private (remove private) computers. Any third-party company e.g. hearing aid manufacturer is only allowed to process your data for the specified purposes and in accordance with our instructions.

We retain your information for as long as is reasonably necessary to provide our products and services and to maintain records to satisfy legal requirements.

WHEN WE MAY SHARE YOUR PERSONAL INFORMATION

Where necessary we may disclose your information to other health care professionals including the NHS. If Herts Hearing Ltd is wholly or partially transferred to a third party, your personal information may be one of the transferred assets.
